Please note that all patients are to register at the Health Center registration desk, which is open from 6:30 a.m. to 5:30 p.m., Monday through Friday. Those registering for a procedure on the weekends should report to the Emergency Desk
Urgent Care is still available from 11:00 a.m. to 7:45 p.m. every day in the Health Center. Those patients seeking Emergency Care can go directly to the Emergency Desk at any time.

Consent for Treatment - Authorization form
Anytime you and your child will be apart it's a good idea to leave written permission with your caregiver to assure prompt medcial treatment. Click on the link below to obtain a form which authorizes consent for medical care treatment. Fill it out and give a copy for your caregiver to keep.
